It's a new year - don't forget to complete your 2012 registration forms upon participation of your first activity this year! In order to participate in Friendship Adventures events, we require that all participants and volunteers complete the below Release and Liability Forms. These forms must be completed annually, therefore, a new form will be required upon attendance at your first event each fiscal year.
